Optical Coherence Tomography （OCT）System信息翻译Optical coherence tomography (OCT) system is the latest generation of intravascular imaging equipment which is designed based on advanced photonics and optical fiber technology. It uses near infrared ray to detect intravascular micron-level structures. The resolution is as high as 10-20um, much higher than any existing imaging technique, which is sufficient to meet clinical requirements. It can be used for early diagnosis of coronary heart disease, introducing implementation of interventional therapy and assessing therapeutic efficacy. OCT is manufactured by American Lightlab Imaging Corporation, and exclusively sold by Lepu (Beijing) Medical Devices Co., Ltd in China. 信息翻译Mobile M2CV OCT system cart includes a light source and a computer. On the top of cart there are two monitors, a storage drawer, a keyboard, a mouse and probe incorporation unit (PIU).Operation principle of OCTThe light source of OCT is near infrared light. It uses optical interference between reference light pulse and signal light pulse of detection object to measure structures of samples with different depths. Through longitudinal and axial scanning of probe or sample table, the obtained signal is processed by computer and three-dimensional CT images of the sample are generated.Accessories of OCT system:1. Imaging guide wireThe effect of imaging guide wire is to generate OCT image. Its outer layer is a plastic sheath, containing an optical fiber with a microprism at the front end. On the top of the guide wire there is a 15mm radiopaque spring tip. The maximum outer diameter of the insertable part of the guide wire is 0.019” (0.48mm) and the diameter at transparent sheath imaging window is 0.014” (0.36mm). The working length of imaging guide wire is 190 cm and the total length is 202 cm.2. Occlusion balloon catheter (OBC)Red blood cells in blood absorb infrared light, influencing vascular wall imaging. The effect of occlusion balloon catheter is to eliminate blood from imaging area. The blood expanded at low pressure can block proximal blood vessels in target lesions. Continuous transluminal low-volume washing can clear up blood in target lesions.3. LID-1 balloon pressure meterThe pressure gauge range of low-pressure expansion device for occlusion balloon expansion is 0-1.0 atm, and precision 0.05 atm.The pressure for occlusion balloon expansion during imaging and scanning should be lower than 0.5 atm.Clinical application of OCT1. To reveal morphology and nature of plaquesImage of fibrous plaque discovered in OCT imaging is an abundant homogeneous high-intensity zone; image of fibrous calcified plaque is a low-intensity zone with clear boundary; image of lipid plaque is a low-intensity zone with unclear boundary.2. To identify vulnerable plaquesPathological features of vulnerable plaques causing clinical emergencies mainly include: an atherosclerotic core composed of abundant extracellular lipid and cell debris (larger than 40% of total plaque volume), a thin fibrous cap (usually less than 65um) and massive infiltration of macrophages and T cells. OCT can well identify fibrous cap and lipid core (sensitivity above 90%) and discover plaque rupture.3. Application value of OCT in PCI treatmentOCT can observe poor stent adherence, intimal tear and tissue prolapse. It can assess stent adherence status by measuring the distance between stent beam and vascular wall.4. Application of OCT in PCI follow upReports of late-stage thrombosis caused by incomplete intimal coating on the surface of stent after implantation of drug-elute stent gradually increase. How to detect intimal coating status after drug-elute stent implantation is now the focus of attentions. OCT can observe the status of intimal restoration, providing powerful support in deciding withdrawal time of anti-platelet drugs.Now, the resolution of M2 OCT system is much higher than any existing cardiovascular imaging method.
